Who We Serve
Children and Youth with Special Health Care Needs (CYSHCN) are children who have or are at increased risk for chronic physical, developmental, behavioral, or emotional conditions. They also require health and related services of a type or amount beyond that required by children generally.

As a parent or family member/caregiver, once you contact Indiana Family to Family, you will be connected with a Peer Services Specialist who is an experienced and extensively trained parent of a child with additional health or education needs. Resource Specialists focus on teaching families how to identify child and family needs, access appropriate resources, and navigate community systems to help them meet the needs of their children. We help families understand policy and program rules while providing a listening ear. We know that families may have future needs, and we develop an ongoing connection with you to anticipate next steps in supporting your child and family. Our focus is families with children age birth to 22.
We serve healthcare and education professionals who serve children and families. This may include technical assistance to help professionals understand the systems of care available to a family. Service also includes delivering experience-based training for specific programs or resources, partnering on work groups and committees to inform quality policy, practice and systems of care.
Managed Care Project
INF2F works in partnership with MDwise, MHS and CareSource, Medicaid managed care organizations (MCO), to provide premium service to its member families who have children who require additional health and educational support. Children and youth with chronic illnesses or disabilities often have complex medical, educational, social and vocational needs that require a wide range of services. The number of providers and agencies involved can be overwhelming to the family. INF2F staff are available to assist families in their efforts to understand the medical home concept and to ensure that the child receives appropriate, comprehensive family-centered care.
INF2F provides intensive services to families of children who require additional health and educational support in conjunction with medical and behavioral case management received from our MCO partners. The range of services INF2F provides includes information & referral to community resources, peer to peer training regarding health care financing and education options, and parent to parent support. INF2F also educates the case management personnel about community resources, education, health care financing options and disability issues.
Community Pediatrics Training Initiative
In collaboration with the Indiana University School of Medicine, INF2F is one of three community based organizations that provides community learning experiences for pediatric residents in the Indiana University Medical Education Program at Indiana University Health. The project focuses on populations of children with special health care needs who are defined as children with disabilities and serious chronic illness, children whose parents speak English as a second language, and children who are victims of domestic violence. This project is changing the way pediatric residents are trained to work with community organizations in meeting the needs of children who require additional health and educational support and their families through site visits, interactions with families and children, and special projects.
Indiana Family to Family staff are available to attend resource fairs as well as provide training and resources to professional and family groups throughout Indiana. Please contact us to discuss your group’s needs.
